Service Agreement
- Nature of Services
- The Clinic provides Ayurvedic consultations, recommendations, and educational services designed to promote general wellness.
- The services offered are not a substitute for professional medical advice, diagnosis, or treatment. The Clinic does not diagnose, treat, cure, or prevent any medical conditions.
- The Clinic’s practitioners are trained and hold degrees in Ayurveda and provide services intended for educational purposes only. The information provided by the Clinic is not intended to diagnose, treat, cure, or prevent any disease.
- The clinic doesn’t Prescribe, advise, recommend, administer, or dispense for compensation of any kind, direct or indirect, a drug or medicine, appliance, mold or cast, application, operation, or treatment, of whatever nature, for the cure or relief of a wound, fracture or bodily injury, infirmity, or disease.
- Limitations of Services
- The Clinic does not practice medicine and surgery.
- The Clinic does not conduct physical examinations.
- The Clinic does not perform surgical procedures.
- The Clinic does not practice osteopathy and surgery.
- The Clinic does not practice podiatry and surgery.
- The Clinic does not provide nutritional counseling that involves the diagnosis and treatment of disease.
- The Clinic does not prescribe pharmaceuticals or conventional medical treatments.
- Client Acknowledgment
- The Client acknowledges that the services provided are educational in nature and are intended to complement, not replace, professional medical care.
- The Client agrees to consult with their primary healthcare provider before making any changes to their health regimen based on the recommendations provided by the Clinic.
- Scope of Practice
- The Clinic’s practitioners are trained and degree holders in Ayurveda and provide guidance based on Ayurvedic principles and also certified to teach yoga. The Clinic does not practice medicine and surgery, doesn’t practice osteopath and surgery podiatrist and surgery.
- All recommendations and suggestions made during consultations are based on traditional Ayurvedic knowledge and are not intended to diagnose, treat, or manage medical conditions.
- Use of Information
- Information provided during consultations is for educational purposes only and is not intended to replace medical advice from a licensed healthcare provider.
- The Client agrees to use the information provided at their own discretion and risk.
- No Guarantees
- The effectiveness of Ayurvedic practices and herbal supplements may vary among individuals. The Clinic does not guarantee specific outcomes or results.
- Confidentiality
- The Clinic is committed to protecting the privacy and confidentiality of the Client’s personal health information (PHI). The Clinic will not disclose any PHI without the Client’s written consent, except as required by law.
- Payment and Fees
- The Client agrees to pay the fees for services rendered as per the Clinic’s current fee schedule. Payments are due at the time of service unless otherwise arranged.
- Cancellation and Refund Policy
- The Client may cancel an appointment with at least 24 hours’ notice. Failure to provide 24 hours’ notice may result in a cancellation fee as per the Clinic’s policy.
- Refunds for services will be handled on a case-by-case basis.
- Client Responsibilities
- The Client agrees to provide accurate and complete information about their health history and current health status.
- The Client agrees to follow the recommendations and suggestions provided by the Clinic to the best of their ability.
- The client agrees to Consult with a licensed healthcare provider for any medical conditions or concerns.
- Governing Law
- This Agreement shall be governed by and construed in accordance with the laws of the State of Ohio.
- Forum Selection
- The parties agree that any disputes arising under or related to this Agreement shall be brought exclusively in the courts located in Franklin County, Ohio
- Entire Agreement
- This Agreement constitutes the entire agreement between the parties and supersedes all prior discussions, agreements, or understandings of any kind.
- Severability
- If any provision of this Agreement is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
- Amendments
- Any amendments or modifications to this Agreement must be made in writing and signed by both parties.
